Діагностика й лікування пошкоджень хондролабрального комплексу кульшового суглоба в дорослих (огляд літератури)

Резюме

Хондролабральний комплекс — це з’єднання суглобової губи й хряща кульшової западини кульшового суглоба. Кульшова западина оточена потужним волокнистим хрящем, що називається суглобовою губою. Суглобова губа створює ущільнення навколо суглобової западини, забезпечуючи щільне з’єднання і збільшуючи стабільність суглоба. Гіаліновий хрящ покриває суглобові поверхні й утворює гладкий шар із низьким коефіцієнтом тертя, що полегшує вільне ковзання кісток між собою. Існує багато причин ушкодження хондролабрального комплексу кульшового суглоба, багато з них пов’язані з травмуванням під час занять активними видами спорту. Основною ж причиною ушкодження хондролабрального комплексу є фемороацетабулярний конфлікт. Це патомеханічний процес хронічної травматизації суглобової губи і хряща кульшової западини головкою або шийкою стегнової кістки. Разом з тим результати багатьох досліджень показують, що досить часто ушкодження хондролабрального комплексу кульшового суглоба є наслідком старіння і мікротравм суглобової губи й хряща. У цій роботі проведений аналіз сучасних способів діагностики й лікування пошкоджень хондролабрального комплексу кульшового суглоба в дорослих.

The chondrolabral complex is the connection of the articular labrum and the acetabular cartilage. The acetabulum is surrounded by a strong fibrous cartilage called the articular labrum. The articular labrum creates a seal around the joint cavity, providing a tight connection and increasing the stability of the joint. Hyaline cartilage covers the joint surfaces and forms a smooth layer with a low coefficient of friction, which facilitates the free sliding of bones between themselves. There are many causes of damage to the chondrolabral complex of the hip joint, many of them are associated with an injury during active sports. The main cause of damage to the chondrolabral complex is femoroacetabular impingement. This is a pathomechanical process of chronic traumatization of the articular labrum and the acetabular cartilage by the femoral head or neck. However, the results of many studies show that quite often damage to the chondrolabral complex of the hip joint is a consequence of aging and microtraumas of the articular labrum and cartilage. This paper analyzes modern methods of diagnosis and treatment of damage to the chondrolabral complex of the hip joint in adults.
