Optimization of Pathogenetic Treatment of Secretory Diarrhea in Infants

Резюме

The article provides a literary analysis of the current state of the problem of early diagnosis of tonsillopharyngitis in children. Namely-timely etiological diagnosis of the disease . The etiology , epidemiology , clinical picture , differential diagnosis , paraclinical diagnosis, treatment directions tonsillopharyngitis in children. Emphasis was placed on the literature review and the modern problems of possible prompt etiologic diagnosis of tonsillitis in children , as well as the principles of rational antibiotic therapy and disease prevention methods from the standpoint of evidence-based medicine . Among the bacterial pathogens of importance belongs gemolititichnomu β-streptococcus group A (BGSA) (S.pyogenes), which occurs in 5-30% of cases of acute exacerbations of chronic tonzilofaringita and tonsillopharyngitis . The paper notes that of particular importance early diagnosis of streptococcal etiology tonsillopharyngitis , which allows at the very beginning of the disease to assign reasonable antibiotic therapy . This will reduce the duration of disease significantly reduce the risk of early morbidity and significant reduction period transmissibility of the patient, which in turn reduces the likelihood of spreading of streptococcal infection.
"Gold standard" for detection of β - hemolytic streptococcus for the appointment of antibiotic therapy is streptatest . The sensitivity of this test is , according to multicenter clinical trials of 97.3 % and a specificity of 95.3 %. Modern test systems provide results in 15-20 min. with high specificity ( 95-100 %), but lower than in the study of the culture , the sensitivity ( 60-95 %). However, it should be remembered that express-methods complement , but not replace , the culture method . Furthermore, only in the event of agent can determine its sensitivity to antibiotics. That is, a negative rapid-test does not completely rule out streptococcal etiology. In this regard, for suspected streptococcal throat , despite the negative Express-test specialists American Academy of Pediatrics recommend spending additional bacteriological smear throat .
Using Express-test systems in children''s infectious diseases clinic should lead to improved early etiologic diagnosis tonsillopharyngitis in children and thereby contribute to the timely appointment of adequate treatment , which ultimately should significantly reduce the risk of complications and contribute to a positive economic effect.

Целью исследования была оценка клинической эффективности пероральной регидратационной терапии с использованием смеси III поколения в лечении секреторных диарей у детей грудного возраста. Для этого на базе инфекционного боксированного отделения (кишечных инфекций) областной детской клинической больницы (г. Черновцы) обследованы 116 детей грудного возраста, выбраных случайным методом, страдающих острым гастроэнтеритом, которые поступали в стационар с признаками эксикоза вследствие развития секреторной диареи. Среди обследованных пациентов 73 (67,5 %) ребенка с целью пероральной регидратационной терапии получали регидратационную смесь, а 35 (32,4 %) — другие регидратационные растворы. Проведенный мониторинг динамики состояния пациентов позволил утверждать, что при использовании в качестве основного компонента пероральной регидратационной терапии смеси III поколения положительная динамика клинического состояния пациентов была достоверно более быстрой, в частности достоверно раньше нормализовались температура тела, частота и характер стула, исчезала рвота. У детей, получавших регидратационную смесь, по сравнению с пациентами, получавшими другие регидратационные растворы, отношение шансов ограничиться лишь пероральной регидратацией составило 3,7 (95% ДИ 0,4–38,9) при абсолютном риске избежать потребности в инфузионной терапии — 11 %.

Метою дослідження була оцiнка клінічної ефективності пероральної регідратаційної терапії з використанням суміші ІІІ покоління в лікуванні секреторних діарей у дітей грудного віку. Для цього на базі інфекційного боксованого відділення (кишкових інфекцій) обласної дитячої клінічної лікарні (м. Чернівці) обстежені 116 дітей грудного віку, вибрані випадковим методом, які страждали від гострого гастроентериту та надходили до стаціонару з ознаками ексикозу внаслідок розвитку секреторної діареї. Серед обстежених пацієнтів 73 (67,5 %) дитини з метою пероральної регідратаційної терапії отримували регідратаційну суміш, а 35 (32,4 %) — інші регідратаційні розчини. Проведений моніторинг динаміки стану пацієнтів дав змогу стверджувати, що за умови використання як основного компонента пероральної регідратаційної терапії суміші ІІІ покоління позитивна динаміка клінічного стану пацієнтів була вірогідно більш швидкою, зокрема вірогідно раніше нормалізувалися температура тіла, частота й характер випорожнень, зникало блювання. У дітей, які отримували регідратаційну суміш ІІІ покоління, порівняно з пацієнтами, які отримували інші регідратаційні розчини, відношення шансів обмежитися лише пероральною регідратацією становило 3,7 (95% ДІ 0,4–38,9) при абсолютному ризику уникнути потреби в інфузійній терапії — 11 %.

Introduction

According to UNICEF, each year about 1.5 million children under 5 years die from dehydration associated with acute infectious diarrhea. The severity of AII in children, in mainly, determined by the volume of fluid loss, that’s why the basic principle of management of these patients is the rapid restoration of fluid and salt loss, and increased buffer capacity of the blood. In conducting rehydration therapy preference should be given oral rehydration. Oral rehydration is a highly effective, simple, accessible at home and inexpensive method. Currently, one of the areas of optimizing the composition and properties of solutions for oral rehydration therapy recommended by WHO is to reduce their osmolarity to 245 mmol/l (third generation of solutions).

The aim of the research — to establish clinical efficacy of oral rehydration therapy using solutions of the third generation in the treatment of secretory diarrhea in infants.

Material and Methods

To achieve this aim examined 116 infants, randomly selected, with acute gastroenteritis and were admission to the hospital with signs of exycosis as a result of secretory diarrhea. The mean age of patients was 9.2 ± 0.8 months. Among 73 evaluable patients (67.5 %) children (I clinical group) to receive oral rehydration therapy mix and 35 (32.4 %) patients (II clinical group) received other rehydration solutions. Dynamic assessment of the clinical severity of the infants made within 7 days of hospitalization by constellations of points: 1) violation of the general condition of the patient (3 points — severe, 2 points — moderate, 1 point — mild), 2) the severity of diarrhea (1–3 points), 3) the severity of vomiting (1–3 points).

Results

Used as a major component of oral rehydration therapy mixtures third generation positive trend of the clinical condition of patients showed significantly faster (normalization of body temperature, frequency and character of bowel movements, disappearance of vomiting). Dynamic scoring the severity of the patients examined shows that, starting from the 3rd day of hospitalization, children who received ORT mix differed significantly better clinical performance.

In the case of mixtures for oral rehydration in infants average gain in body weight in them was 103.5 ± 10.2 g to 78.5 ± 8.7 g (P = 0.05).

In children treated with mixture of compared to representatives of the second clinical group, the odds ratio to limit oral rehydration alone (without infusion therapy) was 3.7 (95 % CI 0.4–38.9) with an absolute risk to avoid the need for infusion therapy — 11 %.

Conclusions

1. Use for the purpose of oral rehydration therapy mixtures third generation is characterized by a relatively positive clinical dynamics as a likely shorter term diarrhea, earlier normalization of body temperature, cessation of vomiting and best rates to restore the lost weight.

2. Efficacy of oral rehydration using the increases the chances of avoiding the need of infusion therapy in 3.7 times.
